Do I need to keep my insurance?

Yes. To ensure your access extends beyond our walls and you get the care that’s right for your unique needs, you will need to keep your insurance for when we connect you to local specialized providers and hospitals outside of Avenu Health.

What if I need specialized care or inpatient care?

At Avenu Health, we serve as your guide to navigate the care that’s right for you so you have hassle-free care when, where and how you need it. Through thoughtful, proactive coordination with local specialized providers and hospitals, we make connections for you to ensure your access extends beyond our walls so that you get the right care for your unique needs. If you are hospitalized or have surgery, we’ll also check in on you.

Who can join Avenu Health?

Children and adults up to 64 years of age can join Avenu Health. We believe that the core of primary care and a key ingredient to health and wellness is a human relationship. That’s why you and your family can join. We take the time to get to know you, your values and your family to help you achieve your optimal health.

If you are 65 years of age and older with Medicare, contact us at info@AvenuHealth.com to stay updated on how we are working to create options for you through a simplified payment plan.

How much is a membership at Avenu Health?

Our rates are simple. With our upfront membership pricing, you know exactly what you are paying, every time – no insurance and no secrets.

For You:

  • Adult (age 26-64): $70/month
  • Child/Young Adult (age 0 to 25): $50/month

For You and Your Family:

  • One Adult + 1 Child: $110/month
  • Two Adults, No Children: $130/month
  • Two Adults + 1 Child: $170/month
  • One Adult + 2 Children: $150/month
  • Two Adults + 2 Children: $210/month
  • One Adult + 3 Children: $190/month
  • Two Adults + 3 Children: $250/month
  • Each additional Child: $40/month

How do I schedule an appointment?

With our best-in-class technology platform, you can schedule appointments online, access your health records online and avoid the hassle of completing routine paperwork. You can also call, text or email us – or simply walk into our office. It’s effortless communication on your terms.

Do you accept walk-in, same-day or next-day appointments?

Yes, absolutely. As a client, you have immediate access office visits with walk-in, same-day and next-day appointments for acute, chronic and preventive care. And, when the standard office hours don’t work for your schedule, we will re-work ours. Our providers are available 24/7 via phone, text and video to treat you even in the most inconvenient hours of the day. Telehealth e-visits are also available between 7 a.m. and 9 p.m. daily.

How do I get in touch with my provider outside of normal office hours?

At Avenu Health, we know that your health and wellbeing isn’t limited to only eight hours per day. That’s why our providers are available 24/7 via phone, text and video to treat you even in the most inconvenient hours of the day. Telehealth e-visits are also available between 7 a.m. and 9 p.m. daily.

What does a membership include?

You receive comprehensive care that includes personalized prevention, nutrition and exercise counseling, chronic disease management and care for those occasional sick days.

When you join Avenu Health, your membership includes:

  • Complete physical and wellness assessments
  • Health coaching in behavioral health, exercise and nutrition with healthy cooking classes
  • Chronic disease management
  • Common lab tests, such as blood sugar, metabolic panel, blood counts, urinalysis, pregnancy, flu and rapid strep testing
  • Basic x-rays
  • Routine vaccinations like flu, tetanus and Tdap
  • Minor in-office procedures, such as skin lesion removal, basic suturing and breathing treatments
  • Physician-facilitated care coordination and specialist referrals

You also receive additional perks and reduced prices for services not included in your membership, such as prescription medications, screening and imaging referrals and in-office procedures like joint injections and skin biopsies.
